To Register

To register permanently with the practice you should be resident within the practice area (see map below) and you'll need to complete a registration form and our patient questionnaire.

Proof of Address & Identification Requirements

To help us verify your eligibility within our practice boundary, we request both proof of address and identification where possible.

A driving license can serve as both proof of address and ID. Other acceptable documents include:

  • Proof of Address:
    • Mortgage statement or letter from the mortgage provider
    • Rental agreement
    • Utility Bill
    • Council tax bill
    (Please note: Financial details are not required—only the address on official headed paper is needed. You can present the top portion of the document in person for verification.)

  • Identification:
    • Passport
    • Birth certificate
